Returning your Free Cell Phone / Return Merchandise Authorization Request FormTo return your free cell phone, please fill in the form below and we will send you a Return Merchandise Authorization (RMA) and detailed instructions on how, and where, to send your phone.
Please
remember: All returns must be approved. All returns must be accompanied
by an RMA. All returns must have all original boxes, books, etc in the
same condition as when shipped. You must also contact the service provider
directly (AT&T Wireless) to cancel service. Usage fees may be charged
by AT&T Wireless if you used over 30 minutes of phone time during
the 30-day grace period. You are responsible for return shipping costs.
The best way to ship the unwanted phone back to us is to send it Third
Class Mail, with the Return Merchandise Authorization (RMA) Number, from
the Post Office. You will want to insure the package for the value of
the phone.
